PATHere.org’s Scholarship application package consists of this application form and the required supplemental documentation, as detailed in the Scholarship Procedures. The complete application packet is due May 18th.

Note for questions 11 and 12: PATHere.org’s Scholarship application packet requires two letters of recommendation. One letter should be from a teacher or school counselor and the other letter may be from any non-family member. In sections 11 and 12, please provide complete information about the individuals who will send the letters of recommendation.
